Decision to Meet Face-to-Face
After careful consideration and evaluation, we have made the decision to move forward with hosting our Nutcracker production this year. While we currently plan to host rehearsals in-person at the studio. we understand that there may be a resurgence of the pandemic or if other health concerns arise, we may require some modifications to in-person rehearsals and may need to move to virtual Zoom rehearsals until it safe to return to in-person rehearsals. If you have concerns about rehearsals via Zoom, please feel free to contact us to discuss your concern. While Zoom rehearsals are not our first choice, our priority is maintaining the safety of our dancers.
Risks of Opting for In-Person Services
You understand that by coming to the DBA studio, you are assuming the risk of exposure to the coronavirus (or other public health risk). This risk may increase if you travel by public transportation, cab, or ride-sharing service.
Your Responsibility to Minimize Your Exposure
To obtain services in person, you agree to take certain precautions which will help keep everyone (you, other dancers, our staff, and all our family and friends) safer from exposure and sickness. If you do not adhere to these safeguards, it may result in moving to Zoom rehearsals. Initial each to indicate that you understand and agree to these actions:
We may change the above precautions if additional local, state or federal orders or guidelines are published. If that happens, we will discuss any necessary changes.
Our Commitment to Minimize Exposure
DBA has taken steps to reduce the risk of spreading the coronavirus within the DBA facility, and these steps are posted at the studio. Please let us know if you have questions about these efforts.
If You Are Sick
You understand that we are committed to keeping volunteers, staff, guests, dancers and our families safe from the spread of this virus. If you show up for a rehearsal, in the lobby, or in a Danse Boutique to make a payment or buy dance merchandize, If we feel that you have a fever or other symptoms, or believe you have been exposed, we will require you to leave the building immediately. If you need to make a payment, you can do that through our website at www.festivalballet.net.
If any staff tests positive for the coronavirus, we will notify you so that you can take appropriate precautions.
Your Confidentiality in the Case of Infection
If you or your child have tested positive for the coronavirus, we may be required to notify local health authorities that you have been in the office. If we have to report this, we will only provide the minimum information necessary for their data collection and will not go into any details about the reason(s) for our visits. By signing this form, you are agreeing that we may do so without an additional signed release.
Informed Consent
This agreement supplements the general informed consent/business agreement that we agreed to at the start of our work together.
